Accelerated pregnancy termination increases life expectancy for mothers and neonates in cases of acute fatty liver of pregnancy

Introduction. Acute Fatty Liver of Pregnancy (AFLP) was avrare and catastrophic disease in pregnancy. It usually occurs in the third trimester or post-partum period.
Materials and methods. Population target pregnancy over 26 weeks diagnosed AFLP period from 2010 to December 2016. Data includes clinical manifestations, laboratory result, and pregnancy outcomes. Data divided 2 period, period 1 interval 2010-2013 and
period 2 interval 2014-2016. Differences maternal and fetal mortality were calculated by difference test signifancy p<0.05.
Results. Mean duration treatment was six days in both periods. Symptoms that appear in period 1 most are jaundice, while in period 2 most jaundice, vomiting, and epigatric pain. Acute kidney injury, DIC, metabolic acidosis, glucose intolerance, were frequent complications in period 1. Period 1 mode of delivery of 57% vaginal with a slower termination decision Termination decision was accelerated in period 2 by cesarean section (80%). Maternal mortality in period 1 was 85.7% while period 2 only 40% (p <0.05). Outcome of neonatal 80% survive in period 2, while period 1 survive only 28.6% (p <0.05).
Conclusion. Changed policy to accelerate termination decisions of pregnancy and imediately to deliver by cesareansection decrease the mortality AFLP outcomes.
